(AGING) Adult Day Health Care Healthcare Coordinator (open until filled)
SALARY RANGE: $29,892 - $48,780/Year with full benefits package
HOURS: 40 Hours per week 7:30am to 4:30pm
DESCRIPTION OF WORK:
This position performs intermediate skilled human support work in the support and assistance of senior citizens with personal, social, and health needs, and related work as apparent or assigned. Work is performed under the limited supervision of the ADHC Program Manager and In-Home Services Supervisor.
This position contributes to the development and implementation of client’s service plan, including the healthcare component, along with other staff, family, other service providers and In-Home Services Supervisor. Additional responsibilities include planning and providing educational programs; screening related to health promotion and disease prevention for clients, caregivers and elderly people in the community; preparing various reports on activities and programs; performing administrative tasks associated with program; speaking to groups to inform them of available services and programs; maintaining all required case records that satisfy Medicaid, State funding and agency requirements; providing information and care assistance to individual clients; counseling clients and families concerning medication; administering a variety of medical related services and treatments to clients and assisting with preparation/serving of meals and daily activities and programs as required.
KNOWLEDGE, SKILLS AND ABILITIES:
Thorough knowledge of services, benefits and programs available for senior citizens; thorough knowledge of the demographics of the service area; ability to apply regulations; ability to maintain detailed records; ability to operate standard office equipment and applicable software (e.g. Microsoft Office Professional); ability to solve problems within scope of responsibility; ability to establish effective working relationships with associates and clients.
MINIMUM EDUCATION AND EXPERIENCE:
Position requires a two year college degree in nursing or related field or state certificate and one year of experience working with Dementia related illnesses or an equivalent combination of education and experiencem and a current LPN license for State of North Carolina.
