DEPARTMENT OF AGING AND IN HOME SERVICES
Sampson County Department of Aging is an arm of county government whose purpose is to research, evaluate, implement and administer a coordinated system of services for senior citizens, and in some instances handicapped and very low income individuals, in the most cost-effective manner possible. A client is defined as an individual who is elderly, handicapped, or in certain services, income-qualified. The primary purpose of the Department is to provide services and referrals which will allow a person to live independently in a residence of their choice for as long as possible in their physical and mental state. It is believed that the well being and comfort of the majority of individuals handicapped by age or disability can usually be better and more economically served in their own home rather than an institutional setting. It is not the intent of the Department to replace family responsibilities, but to provide support for them to better carry out their duties to the aging or disabled person(s) in their care.

Sampson County Department of Aging and In-Home Services is accredited through the North Carolina Accreditation Commission and licensed through the North Carolina Division of Facility Services. Staffing includes RNs, certified nursing assistants, and social workers. The services offered by the Department include:

ADULT DAY HEALTH CARE
Sampson County Adult Day/Health Care Center provides care for the aged and disabled adults who need a structured and supervised day program of activities and services. The center offers support and relief to families and individuals providing long-term care; provides meaningful daytime activities to enhance the participants’ self esteem, socialization, and health; and helps participants maintain or regain independence in daily living. This service is available to those individuals aged 60 and above, those under the age of 60 who meet income eligibility requirements, veterans and those individuals approved for the Community Alternative Program (CAP/DA). The center is located in Clinton and is open Monday through Friday, 7:30 a.m. – 4:30 p.m. It is staffed with a full-time manager, 1 full-time LPN, 1 full-time certified nursing assistant, 4 part-time center assistants, and a dedicated group of volunteers. The Senior Center is a multipurpose center designed for the organization and provision of a broad program of services and activities for adults, aged 60 and older, which include: outreach; information and case assistance; health promotion; social, nutritional and educational programs (such as computer classes); volunteer opportunities; and recreational activities. The center is open Monday through Friday, 8:00 a.m. to 5:00 p.m. Transportation is provided. The Center has a full-time manager and 4 center assistants.

In addition, Butler Court Senior Center in Clinton is open from 8:00 am to 2:00pm and offers a variety of activities throughout the week. There are six other Nutrition Sites located in Sampson County. Through these sites, older adults are provided well-balanced congregate or home-delivered meals. In addition to meals, the sites offer participants socialization, crafts and music, and most importantly, a sense of belonging. The contracted meal service is provided by the County.

HOME IMPROVEMENT PROGRAMS
The Home Improvement Program provides labor and/or materials for minor renovations and repairs to dwellings to remedy conditions which are a risk to the personal health and safety of our older adults or to enhance mobility for functionally impaired individuals. Eligible individuals must be aged 60 and above. Specific renovations and repairs include: installation of interior or exterior wheelchair ramps, installation of grab bars and other safety aids, widening of doorways, repair/replacement of windows, repair of essential sinks, showers, tubs or commodes, and repair of basic heating appliances. IN-HOME AIDE SERVICES
The In-Home Aide Services provide professional and paraprofessional services which assist the individual and/or family with attaining and maintaining self-sufficiency and improving the quality of life. It is the intent of the In-Home Aide Service to assist, but not replace, family members in carrying out certain responsibilities for those individuals needing care or support. In-Home Adie services available are:

1. Home Management/Personal Care (Block Grant) – provides support to individuals and/or their families who require assistance with basic activities of daily living and home management tasks. Eligible participants must be aged 60 or above.

2. Community Alternative Program for Disabled Adults (CAP/DA) – provides services to allow individuals who might otherwise be placed in nursing home facilities to remain at home. Eligible participants must be living at home or be in the process of being discharged from a nursing home facility, must be eligible for ICF or SNF care, must be eligible for Medicaid and must require CAP/DA services to remain at home.

TRANSPORTATION SERVICES
The Sampson County Department of Aging is the lead agency for the Sampson Area Transportation Service. The Sampson Area Transportation Service provides transportation to those individuals who are older adults, aged 60 or above and/or disabled adults/children who cannot provide or arrange their own transportation to grocery or drug stores; doctor, dental or kidney dialysis appointments; nutrition sites or adult day/health care; senior games or other older adult activities. top

Community transportation service is also available for those individuals who are transitioning from Work First assistance to the workplace.

The SAT fleet includes: 14 passenger vans, 8 wheelchair lift vans and 4 mini vans. For information, or to arrange transportation services, contact Transportation Coordinator Todd Daughtry at (910) 299-0127.

INFORMATION AND CASE ASSISTANCE
The Information and Case Assistance Program records and tracks information to allow better referrals and followup for incoming callers. The information provided allows the older adult and their family more direct information about programs/services to meet their needs. This service is available Monday through Friday, 8:00 a.m. to 5:00 p.m. at the Office of Aging. For information, contact Angela Faircloth at (910) 592-4653. top

MISCELLANEOUS SERVICES
Legal Services: Legal counseling, living will arrangements and consumer protection assistance are available. Appointments should be made by calling 1-800-682-1016.

Social Security Administration: A representative of the Social Security Administration is available at the Office of Aging on the second Tuesday of each month from 9:00 a.m. to noon. First come, first serve.
